FOR ME
FOR 2010 / 2011 WE HAVE BEEN GIVEN ADDITIONAL FUNDS
FROM NHS EAST LANCASHIRE As a
carer you may be eligible to apply to Carers Link for a small grant from our
Time for Me Fund. The main purpose of these grants is to give carers a chance
to receive the services or equipment that will be of most benefit to them and
enable them to have a BREAK from their caring role. To
apply you need to follow a few simple steps: 1. Apply
to register as a carer with Carers Link. 2. Complete
our assessment process with one of our Carers Development Workers. 3. After you
have been assessed and registered with us as a carer for six months, you are eligible to
apply for a Time for Me grant. 4. Contact Carers Link for our Guidelines
and Application Form. Please read the guidelines carefully and complete the application
giving all requested information. Your application will be anonymised so remember that the information on the
application form is the only information the panel will have when considering
your request. Our Carers Development Workers are available to help you
complete the form so please don’t hesitate to contact them for advice and assistance. 5. Send your completed application to Carers Link and it will be
considered at the next panel. All applicants are notified in writing about the
outcome of their application within a week of the panel date. Please be advised that if you
are applying for a second grant, priority WILL ALWAYS be given to Carers who
have not yet received an award. You must also be aware that awards are NOT
guaranteed and items or services that you have already bought will not be
funded retrospectively. If you do receive an award,
receipts MUST be submitted to Carers Link to comply with the rules from
Lancashire County Council and NHS East Lancashire For further information and an application form
